The pulmonary artery catheter: when and why it should be used.

The pulmonary artery catheter (PAC) has been in widespread clinical use for 20 yr. It has been modified to allow measurement of cardiac output (CO), mixed venous oxygen saturation (Sf, O2), right ventficular ejection fraction (RVEF) and temporary pacing. The indications for insertion of a PAC have multiplied with time, as has the controversy over the contribution (or lack thereof) of the PAC to improving patient outcome. In order to define the role of PAC monitoring in one's practice it is essential to understand both the limitations of PAC-derived information, and the potential risks and benefits associated with using a PAC.
